| Category | Examples | Publication treatment |
|---|---|---|
| Editorial | Typographical error, broken link, formatting, ambiguous wording | Corrected promptly; logged publicly when the change affects meaning. |
| Factual | Wrong name, title, date, amount, calculation, legal status, or source characterization | Primary source rechecked; page, data files, and affected score are updated together. |
| Source or freshness | New filing, leadership change, corrected official page, newer audited report | Review date and freshness indicators are updated; material score changes are logged. |
| Methodology or score | Dimension weighting, NE treatment, inconsistent application, omitted contrary evidence | Recalculated under the published rubric; rationale and any grade change are documented. |
| Organization response | Concise contextual statement or disagreement from the reviewed organization | May be published alongside the review when relevant; it does not replace the evidence assessment. |
Use this form for a specific factual error, outdated source, broken link, calculation issue, missing primary record, or methodology inconsistency. Disagreement alone does not require a change; the supporting evidence controls.
